Each student is prompted to give consent for the IRS 1098-T form electronically upon their first term registration through their My快活林性息 portal.

Electronic Form IRS 1098-T required disclosures:

1. The consent applies to every year the statement is furnished until it is withdrawn and is applicable immediately following the date of the consent.

2. After giving consent, view and print a copy of the Form IRS 1098-T by:

* Login into聽My快活林性息聽portal through the聽听飞别产蝉颈迟别
* Go to the聽Financial Matters听迟补产
* Under the聽My Financial Obligations聽pagelet, select聽1098T Detail Inquiry
* Select the聽Tax Year聽to view the IRS聽1098-T Tuition Statement

3. The consent may be revoked at any time. Written requests may be emailed to聽Student Accounting聽from your 快活林性息 email account. You will receive a confirmation email, indicating the effective date. Paper requests, must include a valid email address for confirmation. Mail paper requests to: 快活林性息, Student Accounting, 18111 Nordhoff Street, Northridge CA 91330-8359. The withdrawal of consent will be effective for subsequent Form IRS 1098-T and will not apply to previously issued electronic Form IRS 1098-T.

4. If you do not consent to receive the Form IRS 1098-T electronically, a paper copy will be mailed to your address on file.

5. You must keep your contact information current. To update your contact information:

* Login into聽My快活林性息聽portal through the聽听飞别产蝉颈迟别
* Under Student Services听迟补产
* Select Student Center
* In Personal Information, select Home Address to access editing area (use tabs to edit personal information)
* Save new information

快活林性息 will inform you of any changes to your contact information.

6. Electronic furnishing of the Form IRS 1098-T will not discontinue until further notice.

7.听Your electronic IRS 1098-t form may be accessed via your student portal. Safari and Apple Web browser are not supported and you will need Chrome, Firefox, or Microsoft Edge to access your tax form.
