How to Apply

  4. 颁辞尘辫濒别迟别听.聽 Meet with your Department of Rehabilitation (VR) Counselor.聽
    1. Don't have VR?聽 Contact聽ncod@csun.edu聽for information or聽.
  5. 贰尘补颈濒听ncod@csun.edu聽that you completed the 快活林性息 application
  6. Initiate registration for services by accessing the聽听(厂础础厂).
    1. Log-in to SAAS using your 快活林性息 User ID (i.e., abc12345) and 快活林性息 Portal password.
      Note:聽Your 快活林性息 User ID is not the same as your Nine-digit 快活林性息 Student ID Number).
    2. Review and accept the Terms of Service
    3. Complete Preliminary Screening Questions to the best of your ability and submit your responses.
    4. Respond to a series of more detailed questions, so that we can learn more about the nature of your request.

    5. If available, submit an audiogram or/and disability documentation (i.e., a note from a doctor indicating your disability, an IEP, Section 504 Plan, or Psycho-Educational Report, Mental Health Form or Physical Disability Form signed by a licensed provider).聽Disability documentation should contain a specific diagnosis indicating whether the disability is temporary or permanent with a concise description of the functional limitations imposed by your disability. All documentation must be legible and provided in English. If the original documentation is in another language, the student will need to provide a translated copy. Students without documentation are invited to discuss their specific situation with a NCOD or DRES聽counselor.

Following these steps聽will give you priority聽registration and make聽you eligible for services聽such as interpreting, transcribing (captioning) and shared聽notes.
