Students can now apply online through聽.

However, we strongly suggest you complete steps 1-5 below prior to applying through聽Admissions and Records.

1. Identify your option advisor by聽
2. Obtain a current electronic copy of your DPR in PDF format that reflects the year you first declared Kinesiology as your major;
3. Obtain and complete your Kinesiology evaluation form
   1. Scroll down this page and select the form that corresponds to your kinesiology option & catalog year;
4. Send your advisor an email and include a copy of your聽current DPR聽(PDF format) and your聽Kinesiology Option Evaluation Form聽(PDF format) as complete as possible (make sure you use the one that is for your option and your catalog year). If changes are required, your advisor will let you know. If your form is complete, your advisor will email you the signed form;
5. Once the Kinesiology Evaluation Form is signed by your advisor, go to聽聽and upload the form.

If your application is late, a $10 late fee will be assessed. Applying late may also prevent you from receiving graduating senior priority registration during your last semester.聽If you do not get priority registration for your last semester, the Department of Kinesiology cannot guarantee your enrollment in the courses you need to graduate on time.
